A PRACTICAL GUIDE TO HOW WE WORK

Frequently Asked Questions

A practical guide to how Ash & Bloom Wellness works. If you have a question we have not answered here, please contact the practice.

About the Practice
What is Ash & Bloom Wellness?
Ash & Bloom Wellness is a private psychiatric practice in Raleigh, North Carolina. We offer integrative, evidence-informed psychiatric care for adults, including medication management and psychotherapy. Our approach considers the whole person: sleep, nutrition, movement, relationships, and meaning, alongside symptoms and medications.
Who do you treat?
We treat adults (18 and older) with a range of mental health concerns, including:
• Anxiety disorders
• Depression and mood disorders
• ADHD
• PTSD and trauma-related conditions
• Bipolar disorder
• Obsessive-compulsive disorder
• Sleep concerns
• Life transitions, burnout, grief

We do not currently see children or adolescents. For youth crisis or psychiatric care, see the Crisis section below or our Crisis & Safety resource sheet for local options.
What does “integrative” mean here?
It means we look beyond a diagnosis and a prescription. We ask about sleep, eating patterns, exercise, substance use, relationships, work, and what gives your life meaning. When the evidence supports it, we may discuss supplements, lifestyle changes, or referrals to therapy, nutrition, or other providers as part of your plan. Medication is one tool among several, not the only one.
Are you accepting new patients?
Yes. Availability changes from week to week. The fastest way to check is to start the new patient booking process on our website or call the practice.
Appointments and Telehealth
How do I book a first appointment?
You can book through our website or by calling the practice. After booking, you will receive an email invitation to set up your Valant patient portal account and complete intake forms.
How long is the first visit?
Your initial psychiatric evaluation is 60 to 90 minutes. You will leave with a working diagnosis, a written treatment plan, any necessary prescriptions, and a scheduled follow-up.
How long are follow-up visits?
Most follow-up visits are 25 to 30 minutes. Longer 50-minute visits are scheduled for medication changes, psychotherapy sessions, or complex updates.
Are visits in person or by video?
Most visits can be done by secure video through your Valant patient portal. Some visits, including the initial evaluation if controlled medications may be considered, are best done in person. We will tell you ahead of time which is needed.
What if I need to cancel or reschedule?
Please give at least 24 hours’ notice. Cancellations with less than 24 hours’ notice are charged $75, and missed appointments without notice are charged $150. Three no-shows in a 12-month period may end the treatment relationship.
Insurance, Billing, and Fees
What insurance plans do you accept?
Our accepted insurance plans include Aetna, Blue Cross Blue Shield, and select additional insurance carriers. We verify benefits before your first visit so you understand your expected copay or coinsurance. Self-pay options are also available, and superbills may be provided for out-of-network reimbursement upon request. If your insurance plan is not listed, please contact the practice to confirm coverage and available options.
How does billing work?
Most claims are processed through our practice’s electronic health record (Valant). A small number of insurance plans are billed through Alma or Headway. If you booked through Alma or Headway, you will continue to use that platform for billing. We will tell you which applies to you.
Do you offer a sliding scale or self-pay rates?
Self-pay rates are available on request. We also provide a Good Faith Estimate before your first visit if you are paying out of pocket, as required by the No Surprises Act.
Why is there a no-show fee?
A missed appointment is time that could have helped another patient. The fee covers the lost slot and helps us keep wait times reasonable for everyone. We send appointment reminders by email and text, and the Valant portal allows easy rescheduling.
Medications
Will I be prescribed medication?
Not always. Your clinician will discuss what the evidence supports for your situation. Sometimes that is medication, sometimes therapy, sometimes lifestyle and behavioral changes, and often a combination. Medication is offered when it is the best fit, not by default.
How do refills work?
Request refills at least 5 business days before you run out. The fastest path is through your pharmacy, which will send an electronic refill request to us. You can also request a refill through the Valant portal. Refill requests are usually processed within 3 business days.
Do you prescribe stimulants for ADHD?
Yes, when clinically appropriate. We require a thorough ADHD evaluation, including review of any prior records and consideration of nonstimulant options. An in-person visit is generally required to initiate stimulant treatment. We follow North Carolina Controlled Substance Reporting System (NCCSRS) guidelines and use a controlled-medication agreement.
Do you prescribe benzodiazepines?
No. Ash & Bloom Wellness does not prescribe benzodiazepines (for example, Xanax, Ativan, Klonopin, Valium). The current evidence shows that benzodiazepines carry significant risks of dependence, cognitive effects, and rebound anxiety, with limited long-term benefit for most anxiety conditions. We focus on treatments with stronger long-term outcomes: SSRIs and SNRIs, buspirone, propranolol for performance anxiety when appropriate, hydroxyzine, and evidence-based therapies including CBT, ACT, and exposure-based approaches.

If you are currently taking a benzodiazepine and want to work with us, we are glad to help you with a safe, gradual taper. If continued benzodiazepine prescribing is essential for your care, we will help you find a provider whose practice model fits.
What about sleep medications like Ambien?
We use Z-drugs sparingly and short-term, only when first-line sleep options have not worked. For most chronic sleep concerns we start with cognitive behavioral therapy for insomnia (CBT-I), sleep hygiene, and medications with better long-term safety profiles.
Do you treat substance use disorders?
Yes. We provide a full substance use disorder (SUD) program integrated with psychiatric care.
• Opioid use disorder (OUD). We prescribe buprenorphine/naloxone (Suboxone) for medication for opioid use disorder (MOUD). We accept new patients for induction as well as transfers already stabilized on maintenance.
• Alcohol use disorder (AUD). Naltrexone (oral or Vivitrol), acamprosate, and disulfiram when indicated, plus behavioral support.
• Stimulant use disorder. Pharmacotherapy per ASAM guidelines when indicated, plus behavioral interventions, contingency management approaches, and relapse prevention.
• Cannabis and other SUDs. Behavioral interventions, relapse prevention, and treatment of any co-occurring psychiatric conditions.
• Co-occurring care. We treat depression, anxiety, PTSD, bipolar, ADHD, and other conditions in the same visits, so you do not have to coordinate two separate teams.

We do not prescribe other opioids for pain. SUD care includes urine drug screening, use of the NCCSRS, and a signed treatment agreement at the start of MOUD.
What about supplements and integrative options?
We are open to discussing supplements with reasonable evidence, including omega-3s, vitamin D, magnesium, and others when relevant. We avoid recommending products we cannot verify for purity or dosing, and we are careful about supplement-medication interactions.
Therapy
Do you offer therapy?
Yes. Some of our visits include integrative psychotherapy alongside medication management. If you are interested in therapy with our practice, let us know at your first visit. If a different therapist is a better fit, we are happy to coordinate with an outside provider.
What therapy approaches do you use?
We draw from cognitive behavioral therapy (CBT), acceptance and commitment therapy (ACT), motivational interviewing, mindfulness-based approaches, and trauma-informed care. The approach is matched to you, not the other way around.
Communication, Records, and Privacy
How do I send a message between visits?
Use your Valant patient portal for non-urgent messages, refill requests, and routine questions. Routine messages are usually answered within 2 business days.
What is the portal not for?
Please do not use messaging for new or worsening symptoms, urgent clinical questions, or any crisis situation. For any of these, schedule an appointment or call the practice. If it is an emergency, call 911 or 988.
How do you protect my privacy?
Your health information is protected under HIPAA and North Carolina law. Our full Notice of Privacy Practices is available in your new-patient packet and at the practice. We do not share your records without your written authorization, except where the law requires (for example, certain safety reporting obligations).
Can my family or partner be involved?
Yes, if you choose. You will sign a release of information specifying who we can speak with and about what. You can update or revoke this at any time.
Crisis and After-Hours
What do I do in a crisis?
Ash & Bloom Wellness does not provide 24/7 crisis coverage.
• Immediate danger: call 911 or go to the nearest emergency department
• Suicidal thoughts or severe distress: call or text 988
• Walk-in crisis (Wake): RI-Wake Recovery Response Center, (919) 975-5396, 24/7
• Walk-in crisis (Durham): Durham Recovery Response Center, (919) 560-7305, 24/7
• Veterans: call 988 and press 1

The full Crisis & Safety resource list, including youth and mobile crisis options, is available on request or in your new-patient packet.
What if I need to talk to a clinician after hours?
We do not have an on-call line. Use the crisis resources above. At your next visit, tell us what happened so we can adjust your plan.
Working Together
What if your approach is not the right fit?
That is a fair conclusion to reach, and we will not take it personally. We will help you find a provider whose practice model better matches what you are looking for. The goal is good care, even when it is not us.
How do I leave a review or share feedback?
We welcome honest feedback. You can share concerns directly with your clinician at any visit, message through the Valant portal, or leave a review on Google or Facebook. If something has gone wrong, we want to hear it from you first so we can make it right.

We are looking forward to working with you.
